School Application & Lottery

To download an application for your child, click here. If you would like the school to mail you an application, please call the school at 332-0754 or send an email, including your name, address, phone number and the grade or grades you are applying for, to application@tapestryschool.org. Deadline for submission of applications for the 2008-09 school year is Tuesday, April 1, 2008. Applications must be postmarked by this date to be eligible for the lottery.

Tapestry Charter School is a free public school, so there is no tuition and no entry exam.

Should there be more applications than available spots, a lottery will be held. This year's lottery for the 2008-09 school year (K-8) will be held on Thursday, April 3, 2007 at 3:30PM. All are welcome to attend the lottery, but it is not necessary to be there. All families will be notified in writing within two weeks of the lottery date.

Tours for potential parents of K-8 students are scheduled in the months of November through March. Tours are intended for parents with students eligible for this years lottery. To enter our Kindergarten program, students must be FIVE on or before December 1. The tours are approximately 1 hour long and start promptly at 9:00am. Because tours are during the school day, they are for parents only. Please do not bring children on the tour.

Health Forms

Health Office Guidelines - 2 page summary of the Tapestry Charter School Health Services. This includes information and procedures regarding student physicals, screenings, illnesses, medications, health education, and when to keep you child home from school.

Immunizations Form - Immunization requirements for school entrance.

Emergency Contact / Health Summary Form - Annual form to give the school nurse up to date health and emergency contact information.

Consent to Share Health Information Form - New regulations require this form be filled out in order for the nurse to share medical information with appropriate members of the school staff.

Medications Guidelines - In order for medication to be administered to a student by school personnel, this form will need to be filled out by the parent and physician.

Physical Examination Form - Bring this with you to your child's phyical exam and have the doctor fill out this form.

Computers

Acceptable Use Policy for Network and Internet - 1 page list of computer do's and don'ts for parents and students to read and sign.
